Per the United Way Partnership Agreement, funded partners are required to report on an annual basis. Reporting deadlines are set based on fiscal year end, which are detailed in the table below. We are currently collecting information from grantees with a fiscal year end of August, September, and December.

If you do not have access to our grants portal, please request access here. Note that if you have access but do not recall your password you can reset it yourself by selecting "forgot username/password" on the log-in page.
Organizations must provide files for the following:
Audited financial statements for the most recently completed fiscal year. Please share unaudited financials if audited statements are not yet available.
Form 990 for the most recently completed fiscal year.
Current fiscal year budget.
Most recent year-to-date monthly financial management reports (please include the statement of activities with budget vs. actual and statement of financial position).
Current Board of Directors roster with affiliations. Excel format preferred.
In addition to uploading the files listed above, you'll be asked to complete a brief Governance and Leadership checklist on the grants portal.
Note: For applicants that are legally structured as a program of a larger entity (e.g. government, school district, university, hospital, etc.) please include financial statements and budgets specific to your program instead of the audited financial statements for the entity as a whole.
